What's actually happening

The lower lid has three fat compartments held in place by the orbital septum — a fibrous membrane that acts as the lid's structural retaining wall. With age, genetics, or both, this septum weakens. The fat then pushes forward, creating the visible 'bag.'

Two important distinctions:

  • Bag vs hollow. A bag bulges outward; a hollow (tear-trough) recesses inward. Many patients have both — the bag sitting above a hollow.
  • Bag vs edema. Fluid retention varies day-to-day (worse on waking, after salty meals, before menstruation). A true bag is anatomically present and stable.

Treatment options

Lower blepharoplasty is the definitive treatment. The choice of technique depends on whether skin removal is also needed:

  • Transconjunctival — internal incision, no scar. Best for fat-only correction with elastic skin.
  • Subciliary — external incision below lashes. Allows skin removal alongside fat correction.

Tear-trough filler can mask mild bags by filling the hollow below them, levelling the surface. Works for some patients; doesn't address the underlying fat herniation.

Conservative measures (sleep, salt restriction, head elevation) help with the edema component but don't change the anatomical bag.

Frequently asked questions

Can you have bags in your 20s or 30s?

Yes — hereditary baggy eyes present early, often with a clear family pattern. These cases are excellent candidates for transconjunctival blepharoplasty because the skin is still elastic and the procedure leaves no scar.

Do creams reduce eye bags?

No cream changes the underlying fat herniation. Caffeine-containing creams and cold compresses reduce the edema/fluid component temporarily. Retinoids and peptides improve skin quality but don't address the structural bag.

Will surgery affect dark circles?

The shadow component of dark circles improves dramatically when the bag is corrected. The pigmentation and vascular components are not affected by surgery.
