Hereditary baggy eyes

Young patients — often in their 20s or 30s — with persistent under-eye bags that have always been there or appeared early. Family pattern is usually obvious. Surgical management is different from age-related bags.

What makes hereditary cases different

Most under-eye bag patients are in their 50s+. A subset, however, develop bags much earlier — sometimes visibly present from late teens — due to genetic predisposition. Features of the hereditary pattern:

  • Clear family history (a parent or sibling with the same pattern)
  • Onset in 20s or 30s
  • Bags present regardless of sleep, hydration, salt intake
  • Skin quality typically excellent — no skin laxity, just fat herniation
  • Often very symmetric

Why early surgery often makes sense

Hereditary patients are often excellent candidates for transconjunctival blepharoplasty:

  • Skin is elastic — no need for external incision or skin removal
  • Result is dramatic — addressing the only issue cleanly
  • No external scar
  • Fast recovery — typically 7–10 days back to social
  • Long-term outcome is excellent because the underlying anatomy is otherwise healthy

One consideration: fat repositioning (rather than removal) is particularly valuable in younger patients — preserving fat for future natural volume loss that hasn't happened yet.

Frequently asked questions

Will my bags come back after surgery?

Properly performed transconjunctival blepharoplasty in hereditary cases gives essentially permanent correction — the herniated fat is removed or repositioned and the orbital septum is not 're-bagging' in any meaningful sense. Some additional age-related changes can occur decades later but recurrence of the original bag pattern is rare.

I'm 28 — am I too young?

Not for hereditary bags. We've operated on patients in their early 20s with severe hereditary bags successfully. The threshold is whether the bag bothers you and whether the diagnosis is clear, not age.
